R.N. Geriatric Care Manager -- Part - full time, flexible, benefits available

We need a mature, centered, patient team player who thrives in complex, challenging, changing family situations. Assess, plan and arrange care for elders at home or in a care facility. Oversee ongoing care and help clients get results from medical and other service systems, thereby providing peace of mind to clients' nearby or long distance families.
Requirements include:
Requirements include: some availability all weekdays and after hour/weekend on call rotation; long-term care in home health or other settings a plus; minimum three years experience in assessment, assisting elders and supporting families through stressful situations; familiarity with East Bay medical and elder care resources; residency in northern Alameda or central Contra Costa a plus; CDL/auto insurance, home computer.
