Cohen Cormier Home Care & Care Management provides geriatric care management primarily in Alameda and Contra Costa Counties. We help families plan and coordinate the care of an older relative at home or in a congregate setting. Assessing and managing complex care needs in times of transition or crisis requires great attentiveness and an unusual set of skills. Consequently, we measure our growth carefully and add just the right personnel at the right time in order to continue serving each client as if they were our only client.
