the ability to live safely on their own, family members often step in
and try to help manage their relative’s care-related needs. These
family members often live at a distance from their older relative, are
very busy with family and work responsibilities, and are unskilled as
care managers. Hence their new role often is more than they can or want

• We usually meet with
the family to assess the care needs and clarify the family members’
goals and roles.
• Then we develop and
present a plan of care for the family’s approval.
• Most families rely on
us to arrange for, coordinate and monitor the service components of the
plan, and to recommend changes in the plan as needed.
• We base all of our
activities on the family’s goals. We report to and coordinate with
family members on a regular basis.
