    This service is for older adults in one or more of the following situations:

    They themselves are in a medical crisis.

    Their primary family caregiver is in crisis.

    Their family lives at a distance.

    Their declining capabilities may call for more support at home or a change in their living arrangements.

    After an initial phone inquiry, we meet with our new client and some family members usually in the client’s residence. There we assess the client’s medical, cognitive and social needs as well as the living environment. Then we prepare and present a plan of care. The elder care manager designs the plan according to such considerations as the agreed upon goals, the needs that are clarified in the assessment, and the financial and community resources available to achieve the goals and meet the unique needs of the elder and family.

    Most often the heart of the plan is to enable the elder to continue to live at home as independently as possible or to successfully transition to either a retirement community or a skilled care center. Usually we are asked to implement the approved plan in collaboration with the family.
