Alternate Living Arrangement

In any circumstance, moving to a new residence can be stressful. CCG is sensitive to the disruption and anxiety that a move causes for someone in the later years of their life.

If our client is to transition to a different living arrangement like an assisted living community, we identify and introduce the client and family to appropriate options. Once the choice is made, we assist them in the application, move-in and settling-in process. We do the same for a move to a skilled care center.

Families often ask us to continue our care management after the move in order to assure quality of care and appropriate participation in social and other activities, and to report back to the family on changes regarding their elder’s condition and satisfaction in the new and different surroundings. The family wants the on-site benefit of our trained, attentive eyes and our familiarity with their elder.
